To sign up for our medical records portal, please call the office and leave us a message
with your name, phone number, date of birth and email address and we will
email you an invitation to sign up for our DrChrono OnPatient portal.
On Patient Portal/Check In Prior to Your Appointment
Prior to your appointment, please click the
button below and login to onpatient. Click the grey button to the right of your
appointment labeled "Check in". Make sure to fill out the reason for your visit,
changes to allergies, medications, medical conditions, and any questions or comments.
